How it works - Most of the questions have multiple choice
answers. You can skip any question you do not want to answer, but the results
will be of greater value if everything is answered.
NOTE: If you cannot find a particular item to select in a list, some lists have a textbox added that you can use to write in the missing item. A good example is in the lists of anti-biotics. If the anti-biotic is not listed, you can enter it in the textbox. These lists have been improved in the database. There are more items listed and more choices of effectiveness ratings. At the end of
the form there is an area to say anything you want.
Important note: Limit text box responses to 5 to 10
words (50 letters and spaces total) and limit text area responses to 5 to 10 sentences (250 letters and spaces total).
Categories - There are 4 categories of questions in the Survey: 1) Personal Data,
(age, location, duration of symptoms, status, etc) 2) Types of doctors/treatment
you have tried (a list of antibiotics, antifungals, with effectiveness ratings.
and an area to type unlisted remedies, 3) Environmental Data (pets, climate, pesticides, etc.) 4)
Theory and Miscellaneous (area for opinions, speculation, etc)
NOTE: There are 7 categories in the Database. You can access the new sections after your survey has been (manually) added into the Database. Allow at least 24 hours before trying to access the Database. An email notice will be sent to confirm that you have full access usually within 24 hours.
IMPORTANT: No information gathered or displayed on this website may be copied or used by anyone for any purpose except the people submitting data for the purpose of learning how and where and why they contracted this disease. The data collected here is for the exclusive use of people with symptoms of "Morgellons" disease as described by the Case Definition at Morgellons Research Foundation or USP (unidentified skin parasite) as described by the National Unidentified Skin Parasite Association. All the information gathered here is kept private. It can only be viewed by people who contributed their data to the database. A username and password will be required to view the results.
You can also use the Fiber Disease Forum for discussion of the Survey, the Database or the Results. Use these to post suggestions, questions or information.
Registration - Please create a username and password here before doing the Survey. After completing the registration, you will be re-directed to the Survey. At this point, you can either continue with the original survey (faster) or jump ahead and go directly to the "Expanded" Survey (more detail). To do this you need to login and add data to each section of the database. After logging in, the menulist on the left will now have several "Data" sections. As you click on each section, you see "No Records Found". Click on the "Add" link and proceed thru each section. If there is alraedy a record, click on "Edit", do NOT add another record.
WARNING - It is best to use Internet Explorer 6 for the Survey.
