21 May 2007 - New data maps are in process. Here is one showing number of people per zip code. The colors indicate various ranges. I am working on some other ways to display the data including a 3D map.
This will show the huge numbers within certain areas better.
20 March 2007 - The purpose of this survey has been to collect symptoms, remedies and (probably the most important) Theories about what this stuff is, how it is spread where did it come from, etc. The Theories have covered a wide range possible sources. A few fairly common theories are: genetically altered nematodes or fungi, either
accidentally or deliberately created and released, collembola (springtails), genetically altered cotton that continues growing after being made into
clothing, or the result of biological and chemical aerosol spraying from high altitude jet aircraft. I have always considered every theory possible, that is to say, I never dismissed any theory at face value, without any research, but I did put some farther down the list. After
investigating many of these possible explanations, I can say one thing is definitely true: "Science" has become the leading cause of death and destruction of life. I have read about the development of cotton that resists insects, genetically altered nematodes that virtually cannot be removed from the environment, fungus developed that cannot be killed with
fungicide, etc. Companies eager to get ahead of the competition or attempting to overtake the competition use whatever the scientists have come up with without knowing or asking or caring about collateral damage. Anti-biotics that help to
develop stronger drug resistant diseases, development of a synthetic biological agent for the Department of Defense to which no natural immunity exists, vaccinations that produce epidemics and others make it easy to believe the most unbelievable. I recommend that you do your own research, check out stories and see what is propaganda and what is probably true. I say "probably true" because it will be a long time before the perpetrators of these crimes will allow incriminating
evidence to be released.

09 March 2007 - I have found a very effective treatment and it is worthy of recommendation. I want feed back from everyone who tries it and if you have already used it, please tell me about it. 08 March 2007 - Added the DOP Doctor List. It is small but I hope that everyone who has had bad experiences with doctors will help others avoid them. There is a Good Doctor List but no one has been listed yet.
16 February 2007 - I have closed the Discussion Forum and will be using The Fiber Disease Forum to discuss anything relating to this Survey and Database. I think there are enough Morgellon forums, we don't want to have duplicate discussions spread out all over the internet. By minimizing the number of Morgellon related forums, it will be easier to find information. So this site will contain a Survey, a Dynamic Database and Graphical & Statistical Displays of the data collected. Also a Links Page and maybe a FAQ/Help section if possible
22 December 2006 - If you tried to Register (and do the Survey) but got an error message and did not get redirected to the Survey, you can login find a link in list on left side. After I get the survey data into the database, you can login and do the expanded survey sections.
02 December 2006 - Password Recovery and Changing is now fully functional.
It took awhile to convince the web host that they were responsible for the
malfunctions, but finally, they did take the blame and fixed it.
Another issue I have been dealing with is about a dozen
people did the Survey twice. I want to keep the Results as accurate as
possible, but I don't want to just delete every duplicate. Another effect
of having multiple entries with the same email is the Password Recovery will
only send the first username and password that match the email address.
Now that the problem of duplicates is resolved, I will be
adding another section exclusively for the large text entry parts of the survey,
such as the "Most Unusual Manifestations" and the "Theories & Speculations".
And as soon as enough people do the New "Good Doctor/Bad Doctor" section, I will
start a new results page where you can look for a good doctor nearby and avoid
the ones that are doing snap DOP diagnostics.
15 November 2006 - So I got the Database site working and I send out email to over 300 people and then find that the server has been changed to prevent SPAM and it also prevents the password recovery and password changing functions from working for most if not all users. So you can still login but probably cannot change your password today. If you cannot remember the username you created, email me and include the email address you used when you did the survey. You can login and update your survey data and there are new questions in every section of the survey, too.
09 November 2006 - Almost all of the original survey is accessible thru the database. I am going to send out email notifications to tell everyone to login and update their data. Also there are new things that I think are going to be very useful, esp, the "blacklist/whitelist" of doctors that have been either very useless (or worse) or very helpful. I have to start working on the pages that will display the new data so I will probably not be working on the user access part of the database as much. There are a total of 7 sections now: 1) Email & Privacy 2) Personal Data 3) Symptoms 4) Doctors 5) Treatment 6) Environment 7) Medical (history). I want to see how much interest there is for updating and adding data before I work on it any more. It has been a tremendous amount of work, but I don't have any problem with continuing with it as long as it takes. I have not heard about anyone finding a cure yet.
25 September 2006 - I will be moving the entire Survey and Results sections from the Comcast website onto this site (www.usp-db.com) very VERY soon. I have been saying "soon" for about 10 months but now it will actually be happening (relatively) "soon". The only reason that I have not moved the Results to this site is that I have to use a different method of access control and the Comcast site was very easy for a medium level of security for Results but not adequate for the individual record access I wanted to have. So the New Survey is a high priority BUT protection of personal information, especially of those who already have had to endure the abuse and ignorance of medical "professionals", is the absolute highest priority.
12 September 2006 - There is a new page added to the updatable sections for detailed doctor and DOP info. You can now specify how many of each type of doctor you have gone to AND how many of each type of doctor labeled you "DOP". Also, you can list doctors that were good, Morgellon aware, and you can also list doctors that should be avoided. You should probably ask permission to recommend the Good Docs first. The Bad "DOP labeler" Docs probably do not want to be labeled themselves so don't bother to ask them. I expect that the list of DOP Docs will be huge. I think it is a very good idea to create a list to minimize the waste of our time and money on these ignorant doctors. Also new is the expanded selection of responses available in the Symptoms Section. Now you can choose "Always", "Frequently", "Occasionally", "Rarely" or "Never". So Login to Update Your Record and if it is your first time, use your survey username for both "username" AND "password" and then create a new password. If you cannot remember your username and/or password, you can have it emailed to the address you gave in the survey. Then check out each section of the database. Not all of the survey data is editable but the new expanded Doctor & DOP section is online.
25 June 2006 - FINALLY... www.usp-db.com is up and running. The
user registration and access control is online. You can login and update
the "Personal Data" section of the Survey which includes important time related
data such as how long symptoms have been apparent, current status and work
status. I am now using date of birth (DOB) and onset dates so that the
data displayed is always accurate. Note: exact DOB is not required, just
year, month and any day of the month, if you are concerned about anonymity.
The DOB will not be visible anyway.
Also, you will find a new feature called "Visibility".
To accommodate the wide range of privacy concerns I have added this option to
select how visible your data is. Some people have said they want their
data, including email and even phone number to be publicly visible while
others would like only their email visible to other people in this database and
others who do not want their email address or even their data visible to other
survey participants. Each of these levels of visibility and others in
between these can be selected. This will be used for a reference when
publishing results, sharing data with researchers and helping people connect
with others in their area.
19 February 2006 - The new web site (www.usp-db.com) is 75% functional now. The restricted pages required a different type of authorization which is much more versatile but it took me awhile to get it working. To get into the Survey Results Pages you now use the actual username that you created when you did the survey and you also use it for a temporary password. You cannot change the password yet but I am working on that AND I am working on database accessibility that will do a few things: 1) allow viewing & editing of a survey record by using a username/password, 2) inserting First Survey data into the New Survey when it is online, 3) enabling communication with anyone else who allows their contact info to be visible (password required). You will be able to control whether or not the information (email, phone number, etc) is visible to anyone else that did the survey or just the people nearby or not visible at all. I have always tried to protect privacy as much as possible while also making this database as accessible and useful as possible to the people that have contributed their personal data.
I have been avoiding checking my email recently because I need to work on the restricted area and authorization method of the new site, the creation of an secure online database, the new survey, integration of new survey with old survey data and the database, etc.
11 February 2006 - The posting is back and already new surveys started coming in again. Thanks to LymeBusters Forum for Morgellons, the Survey is visible again!
06 February 2006 - Regarding the posting of my thread about the survey at LymeBusters, I was mistaken when I said that it had been put back or re-pinned in the Morgellons topic area. I have been communicating with Kerry at LB during the past 2 weeks trying to get it back in the Morgellons area without success. Today I sent another PM to her and Patti also. Before the thread was moved to an entirely different area of the LymeBuster forums, I was receiving new surveys at a rate of 5 surveys every 4 days. It has been a very steady rate. During the past 2 weeks, only 1 new survey has come in. The Morgellon topic area is extremely busy. I saw 19 guest viewers there today and I know some of them would probably are new visitors that would do the survey if they knew about it. If you want to help, you can email or PM Patti or Kerry at LymeBusters. I am also going to see if I can get a survey announcement posted at some other locations. 03 February 2006 - New Improved USA USP/Morg Map have much more functions working in the map now. Hovering on a marker shows a little popup with some info and clicking on a marker takes you to a small table of data for each location. Clicking on each state will show statistics for that state.
28 January 2006 - New USA Morg Map. I have more
functions working in the map now. Clicking on a marker opens a page with a
small table of data for each location. I have been adding "Required"
to certain fields in the survey such as "Age", "City", "Symptom Duration", etc.
because when any of these are skipped, the data is much less valuable.
Fortunately, very few people have skipped these. I don't know if they
skipped the question or if they just didn't see it, but now a reminder message
pops up when the SUBMIT button is clicked. A few surveys have had to be
deleted from the database because of errors or missing information that could
have been corrected if I could have contacted the people who submitted them.
So, although an email address is still not required, I have added a requirement
to type SOMETHING in the email field and a detail explanation popup message that
works like this: Click this.
Also, today the total number of surveys hit 200! New surveys
have been coming in at a fairly steady rate of about 5 surveys every 4 days.
However, recently a period of over 5 days went by without a single new survey. I
had no idea why until I visited LymeBusters Morgellons topic listing and saw
that all of the "pinned" topics had been unpinned. I requested that the
survey thread be pinned again and although I haven't gone to LymeBusters to
check, I think it is pinned because a new survey came in today. Thanks
again very much Kerry and all at LymeBusters!
20 January 2006 - The USP/Morg Case Location Map has been improved. Now the location points are accurately plotted on the USA

14 January 2006 - The Profession/Work
Status Table is as compatible as I can make it. If you have
Windows XP with Office XP or Office 2003, you can use links on the page to get
the Web Components if you don't already have them. If you have Windows 98
or ME with Office 2000, there is a link to a version for you also. If you don't have MS Office at all, you can go to the Non-Interactive version page, copy the table and
paste it into a blank spreadsheet and then sort and filter the data.
10 January 2006 - All pages updated including "Unlisted Symptoms", "Manifestations" and "Theories" pages although I didn't spell and grammar correct them as I have been doing. Also, the "Profession/occupation" interactive page should be working. It is a bit tricky to get uploaded and it's not very user friendly but it does work. I do not know if it requires that you have Excel on your computer. Anyone who wants to give me feedback on this issue should email me with this link Worklist Page Report. The "Profession/occupation" page is not updated because of the difficulty in getting a good upload. (Sometimes it's over 2 Megs and won't work, it is about 155kB now and it works). I apologize for not getting the pages about treatments back online yet. There is a lot of data there and it is a lot of work sorting it out and displaying it in a useful way. (if anyone wants to send me some Adderall, I'd be much obliged J/K)
06 January 2006 - You want to see the Entire Database?
03 January 2006 - I added a couple new pages including
an interactive table. It is not password restricted and I hope that people
can see the potential of this database and want to contribute their personal
data. The Profession/Work Status Table has columns for AGE, GENDER, LOCATION,
PROFESSION, and WORK STATUS. Each column can be used to sort and/or filter
the records to display any correlations in the data.
I am working hard to get the new survey ready but I am also
working on the existing database and also still repairing the pages that were
corrupted somehow. Pages 1, 2, 4 and the Theory, Manifestation and
Unlisted Symptoms pages have all been repaired and expanded. Page 1 has
gotten very big.
There is also a new page with charts showing an interesting
correlation between USP/Morg cases per state and "non-white" population
percentage (US Census 2000). Although the survey shows almost 90% of USP/Morg
cases are "white", the highest percentages are in states with highest
percentages of "non-white" population. e.g. California is 37% non-white and
ranks #4 in non-white population percentage (after Hawaii, Washington DC and
Mississippi) and yet has highest number and percentage of USP/Morg cases.
Those are facts.
Now, my comment: The government was
slow to respond to the disaster in New Orleans and the people most hurt by
Katrina were "Non-white" and now here in California with a large non-white
population and the largest number and percentage of USP/Morg cases, again the
government is inexcusably slow to react. What's up with that??!!

29 December 2005 - I am back after a couple weeks of
illness. I Haven't been able to update the results or even collect survey
data due to either allergic reaction or side effects of Bactrim. It has
helped my skin symptoms but I had horrible pulsing pains in my head near the ear
for days then pain in my shoulder like a pinched nerve but no position relieved
pain and couldn't sleep for over 2 days. I also had chills and severe
stomach cramps. Now I stopped the Bactrim and feel good except my left arm
doesn't work well. It acts weird, twists in odd ways when I reach
and no strength. I don't know if it's over yet.
Anyway, I finally updated the "Theories..." &
"Manifestations..." & "Unlisted Symptoms" pages. The first 2 pages of
results are working well, but the other pages are still un-repaired.
I have however added some details to the first page that are interesting.

being updated temporarily for technical reasons. Results Pages 1 & 2 are updated
daily as usual and the "Theories" and "Manifestation" pages are updated about
every 7-10 days as usual.

14 October 2005 - I have put the results pages in a password protected directory because I believe that allowing anyone to view the results affects how they respond to the survey questions and some may not do the survey either because they think their input is not significantly different from what they see in the results or they just don't want to bother with submitting their data. Out of respect for everyone who has contributed their personal information to be shared and compared with others, hopefully for our mutual benefit, I think that the privilege of examining the data should be restricted to people that have participated in building the database. I will be emailing a password to everyone who has already done the survey AND gave me an email address OR used the update notification AND did the survey. I believe the easy access to the results was possibly a factor in the decrease of new surveys recently. So I hope this helps and I hope the survey itself helps everyone in some way.
11 October 2005 - Finally got to the last section of
the survey results display. However, there are still more data views to
come. I am working on a U.S. map view that groups states into regions and
displays the numbers of people and duration of infections graphically. If
the USP is spreading out from an origin point or points, and is airborne, it
might be possible to see in the map view. There are still over 20
states that have not yet been "heard" from in this survey. Even after
adjusting the proportions for the populations of each state, some states seem to
be "under-represented". There are other factors that affect the
probability of someone finding this survey that vary between states, but if this
trend continues, it may be a significant clue.
There are lots of Question Suggestions coming in and I want
to get the "expanded" or "appended" survey online ASAP. Probably, it will
be a few weeks from now, when the current survey results display is pretty much
fully functional and fully automatic. I am thinking of actually
putting the database online so everyone can sort and filter the data anyway they
want. Before I do that, I want to be sure that it's secure and anyone can
edit, block or remove their own data and I also want to limit access to the
people who have helped create the database.
06 October 2005 - Created a new page for survey question or topic suggestions. Go to the New Question page. I am removing the Discussion Forum because there are other forums with much better functionality already. Although NUSPA is apparently not online, there is LymeBusters Morgellons section and a few more (Check the Links Page)
21 September 2005 - Second and third results pages online. Third
page is too complex to display all data in one view so I am working on popup
windows to display comments for each listed item. Currently several
antibiotics and antifungals are listed with ratings. Some of the comments
and details sections are directly copied from the text entries of the survey and
are unfiltered, unedited and unsorted. Although there are redundant
listings, spelling errors, etc., there is some good information to be found.
01 September 2005 - Started displaying the Comprehensive Survey results.
The first stage is "Age", "Gender", "Location" and "Symptoms", etc. The
page will be updated daily. The next stage should be viewable in about a
week and will cover a larger chunk of the survey. You will find links to
the results pages on the "Confirmation Page" after the survey. Completing
the survey is not required to view the results, however it is recommended to do
the survey first.
If you want to get an email when the dB Form results are all online and when the expanded survey is ready, go here: Notification of Database Form updates to get on the address list. However, I suggest that you put your email address in the dB Form to get on the private list. It's easier.
